USI – UNIQUE STUDENT IDENTIFIER

From 1 January 2015, CTQ Construction Trade Qualifications can be prevented from issuing you with a nationally recognised VET qualification or statement of attainment when you complete your course if you do not have a Unique Student Identifier (USI). In addition, we are required to include your USI in the data we submit to National Centre for Vocational Education Research Ltd (NCVER). If you have not yet obtained a USI you can apply for it directly at http://www.usi.gov.au/create-your-USI/ on computer or mobile device. Please note that if you would like to specify your gender as ‘X’ you will need to contact the USI Office for assistance.

All students undertaking nationally recognised training delivered by an RTO (Registered Training Organisation) will need to have a USI (Unique Student Identifier )

FEES AND CHARGES

Please refer to our Student Handbook and www.ctq.edu.au for the full details of our Fees and Charges information.

Process for Smart and Skilled Student Fee determination and approval.

Using the information contained within this enrolment form, CTQ will enter this information into the NSW Smart and Skilled Notification portal, this will create

1. Eligibility Enquiry Report –a Quote will be generated from Smart and Skilled Training market and sent via email for your approval.

2. Notification of Enrolment / Commitment ID – once the Quote has been accepted, we will proceed to Notification of Enrolment and a Commitment ID will be generated from Smart and Skilled Training market along with a copy will be kept on your file.

3. Once the Commitment ID is created, we shall arrange a date/time for a Workbased induction, or induction shall occur on Day 1 of your classroom-based program.

4. 7 days after training has commenced, an invoice for the student fee will be raised and sent via email with payment instructions.

Discontinuing Students – students who have withdrawn, cancelled or been ceased traineeship/apprenticeship employment.

When a Student withdraws from a Training/Course after the Census date (known as the Commencement date of training) the following will apply:

Withdrawal without Penalty

Construction Trade Qualifications will refund/credit any fees paid, without incurring an administration fee, if you wish to withdraw from the course /qualification prior to commencement (Census Date).

You must provide in writing, your withdrawal notice including your reasons for withdrawal to Construction Trade Qualifications, if you wish to withdraw at any time during the course.

Withdrawal after Commencement

If you choose to withdraw post Commencement of your training program (Census date) fees will incur, based upon commencement of UOC's in your qualification/course. The Portion of the Student fee that is to be paid will be calculated based on Total Student fee / Number of Units within the qualification x the Number of Units Commenced.

Eg: Student fee $1000

10 units within qualification = $100 per unit

6 units commenced = Student fee payment $600

The balance due or refund payable will be outlined in writing via email to the student.

· Termination of enrolment does not provide for a refund or waiver of fees.

· Students are not eligible for a refund for any units of competency that have already been commenced at the time of withdrawing from the training under any circumstances other than those referred to under Consumer Protection Refunds.

· Extenuating circumstances which can be evidenced and verified may lead to a refund or course extension up to twelve months free of charge.

Deferring Students

A Student may defer their training/course for up to 12 months from your initial enrolment. If a student returns to training within that 12month period a new Smart and Skilled Fee will not be charged.

If you do not return to your training/course within that 12 month period, you will be reassessed via NSW Smart and Skilled to determine the appropriate fee at the time of your new enrolment.

Where a student is unable complete their Training/Course due to Provider closure or where the provider is unable to continue to deliver Smart and Skilled a refund based on % of qualification yet to be completed, will refunded.

NOTE – Withdrawal from a course/training program or Traineeship/Apprenticeship must be made in writing to

Fee for Service Program Delivery, the following will apply:

· Learners who give notice to cancel their enrolment more than 10 days prior to the commencement of a program will be entitled to a full refund of fees paid.

· Learners who give notice to cancel their enrolment less than 10 days prior to the commencement of a program will be entitled to a 75% refund of fees paid. The amount retained (25%) by CTQ is required to cover the costs of staff and resources which will have already been committed based on the student’s initial intention to undertake the training.

· Learners who cancel their enrolment after a training program has commenced will not be entitled to a refund of fees.

Understand and agree that, under the National Vocational Education and Training Regulator (Data Provisions Requirements) Instrument 2020,Construction Trade Qualifications is required to collect personal information (information or an opinion about me), collected from me, my parent or guardian, such as my name, Unique Student Identifier, date of birth, contact details, training outcomes and performance, sensitive personal information (including my ethnicity or health information) and other enrolment and training activity-related information (together Personal Information) and disclose that Personal Information to the National Centre for Vocational Education Research Ltd (NCVER).

My Personal Information (including the personal information contained on my enrolment form and training activity data) may be used or disclosed by Construction Trade Qualifications for statistical, regulatory and research purposes. Construction Trade Qualifications may disclose my personal information for these purposes to third parties including:

· School – if I am a secondary student undertaking VET, including a school-based apprenticeship or traineeship;

· Employer – if I am enrolled in training paid by my employer

· Commonwealth & State or Territory Govt departments and authorized agencies, including the NSW Department of Education (Department);

· NCVER;

· Organisations (including the Department) conducting student surveys; and Researchers

Personal information disclosed to NCVER may be used or disclosed for the following purposes:

· Issuing a VET Statement of Attainment for VET Qualification, and populating authenticated VET Transcripts

· Facilitating statistics and research relating to education, including surveys;

· Understanding how the VET market operates, for policy, workforce planning and consumer information; and

· Administering VET, including program administration, regulation, monitoring and evaluation.

I may receive an NCVER student survey which may be administered by an NCVER employee, agent or third party contractor. I may opt out of the survey at the time of being conducted.

NCVER will collect, hold and disclose my Personal Information in accordance with the Privacy Act 1988 (Cth), the VET Data Policy, and all NCVER policies and protocols (including those published on NCVER’s website at www.ncver.edu.au)

The Department may disclose my Personal Information to other Australian Government agencies, including those location in States and Territories outside New South Wales.

The above government agencies may use my Personal Information for any purpose relating to the exercise of their government functions, including but not limited to the evaluation and assessment of my training, the determination of my eligibility to receive subsidised training or any Fee Exemptions or Concessions.
